Understanding Ectropion and Entropion: What Causes Eyelids to Turn Inward or Outward?
Eyelid malposition, where the eyelids turn inward (entropion) or outward (ectropion), can be caused by a variety of factors, including:
Ectropion
This condition, where the lower eyelid turns outward and exposes the inner surface, can be caused by aging, facial nerve paralysis (Bell's Palsy), previous eyelid surgery, skin cancer removal, or trauma. Those with prominent eyes, such as patients with Thyroid Eye Disease, are more likely to see their eyelids turn outward.
Entropion
Entropion, the inward turning of the eyelid, often the lower lid, is commonly caused by aging and the relaxation of eyelid muscles. Other causes can include scarring from trauma, infections, or previous surgery like cataracts or retinal surgery.

Recognizing the Symptoms
It’s important that you’re aware of the ectropion and entropion symptoms so you can take action as needed.
Ectropion Symptoms
Ectropion often leads to excessive tearing (due to the tear drainage system being disrupted), dry eye (due to increased tear evaporation), irritation, redness, a gritty or foreign body sensation, and increased sensitivity to light and wind. Some patients may also notice that their lower eyelid appears visibly droopy and pulled away from the eye.
Entropion Symptoms
Entropion's primary symptom is eye irritation caused by the eyelashes and skin rubbing against the cornea. This can lead to redness, tearing, a feeling of something in your eye, light sensitivity, and mucous discharge. Entropion can cause corneal abrasions or ulcers in severe or untreated cases and can potentially impact your vision. Your lashes may rotate inwards, causing the ocular surface to become infected.
Surgical Solutions for Ectropion and Entropion
Dr. Ahmad provides advanced surgical techniques to precisely correct both ectropion and entropion. In our approach, individualization is at the forefront of what we do. He tailors the procedure to your specific condition and the severity of your eyelid malposition. Surgical options may include:
Ectropion Surgical Techniques
Depending on the cause and extent of ectropion, Dr. Ahmad may employ techniques such as tightening the orbicularis oculi muscle (the muscle surrounding the eye), using a skin graft to support the eyelid, or repositioning the eyelid to its normal anatomical position. However, Dr. Ahmad believes that just because you have this, does not mean you need to have surgery. Many patients’ eyes do not get damaged by the lower eyelid pulling away, and there are simple, non-invasive treatments available.
Entropion Surgical Techniques
Entropion repair may involve tightening the eyelid's retractor muscles, removing a small section of skin to reduce inward turning, or strategically placing sutures to correctly position the eyelid against the eye.
