What Is Ptosis? Understanding the Condition.
Ptosis (the “P” is silent!) is a condition characterized by a drooping or sagging upper eyelid. It can affect one or both eyes and may be present from birth (congenital ptosis) or develop later in life (acquired ptosis). In some families, we see adult onset droopy eyes from progressive muscle weakening, generation after generation. These patients need a different type of surgical approach, called a frontalis muscle approach.
The severity of ptosis can vary significantly; some people experience a slight droop while others experience the complete closure of the eyelid. It’s vital to note that the symmetry of your eyes (including the eyebrows and lids) is dependent on the visual system. Our brains make decisions that we can’t always understand or control, but our staff is here to help clarify this complicated issue.Â
Ptosis is typically caused by a stretched-out lifting muscle of the upper eyelid, a heavy or drooped eyebrow, extra fat or skin on the upper eyelids, or a combination of any of these three things. A stretched-out levator muscle is by far the most common cause.


Who Is a Candidate for Ptosis Repair?
Ptosis repair surgery can be a beneficial option for a wide range of individuals, but keep in mind that not everyone will be the ideal candidate for this procedure. Dr. Ahmad will always take the time to discuss your concerns and determine if you’re an ideal candidate for ptosis repair. Generally, this procedure is ideal for those who:
- Are in good overall health and can undergo a surgical procedure.
- Have realistic expectations about the surgery’s outcome. It's important to understand that while ptosis repair can significantly improve the appearance and function of your eyelids, it may not completely eliminate all signs of aging.
- Are bothered by the cosmetic appearance of their drooping eyelid(s). Ptosis can make you appear tired, older, and even sad.
- Experience functional limitations due to ptosis. If drooping eyelids obstruct your vision, particularly your upper field of view, ptosis repair can improve your eyesight and quality of life.
- Have not experienced improvement with non-surgical treatments. In some cases of mild ptosis, we may consider non-surgical approaches. Causes of Ptosis: Why Do Eyelids Droop?
Understanding the underlying cause of ptosis is essential for determining the appropriate treatment approach.
Congenital Ptosis
This type of ptosis is present from birth and is typically caused by an underdeveloped levator muscle, the muscle responsible for lifting the upper eyelid. Patients with congenital ptosis can have severe dysfunction of the muscle and need a frontalis sling, while others have mild ptosis with mild muscle dysfunction and can be fixed with a small Mullers muscle conjunctival resection.
Acquired Ptosis
Acquired ptosis develops over time and can have several causes:
- Aging: The most common cause of acquired ptosis is the natural aging process. As we age, the levator muscle can weaken and stretch which causes the eyelid to gradually droop.
- Muscle Diseases: Certain neurological or muscular conditions, such as myasthenia gravis or Horner's syndrome, can affect the levator muscle's function and cause ptosis.
- Eye Injuries or Surgeries: Trauma to the eye or previous eye surgeries can sometimes damage the levator muscle or the nerves that control it, leading to ptosis.
- Contact Lens Wear: In some cases, long-term contact lens wear can stretch the eyelid muscles and contribute to ptosis. Long-term hard contact lens wear always causes some level of ptosis.Â
- Tumors: While rare, a tumor affecting the eyelid or surrounding structures can sometimes cause ptosis.
